Please be sure to read and fill out the form thoroughly. ID will also be required with the form. Goverment or State ID ONLY. If it's not accepted for Beer, we will not accept it either. Necessary Documents for Tattoo or Piercings

Notarized Consent Form: We have the forms to fill out here or you can download from the link below. It can be notarized at any bank or Check Cashing store.​

Minors ID: Must have a recent Government or State ID for tattoos. A School ID will only be accepted for piercings. We will not Tattoo a minor under the age of 16 or pierce any minor under the age of 15.

Parents or Legal Guardians ID: Government or State ID only. State requires proof of gaurdianship. A birth certificate must be brought in to show that. Legal Guardians must supply proper documents that prove they have legal guardianship.
